Your teacher will sign your application.

Either:
Complete the form electronically then print.
Or print the form and complete it by hand.
Sign your printed form by hand. (Electronic signatures not accepted.)
Give your form to your Christian Science teacher.
Ask your teacher to sign and mail your form to the Clerk of The Mother Church or scan and email it to clerk@christianscience.com.
That’s it! Once we’ve reviewed your application, you’ll be considered for the next admission of members, either in May/June or November.
