Use Special Form if:

• You have had Primary class instruction with a teacher of Christian Science.
• Your teacher will not be signing your form for any one of the reasons listed in the Manual of The Mother Church.

Instructions

• Download the PDF form.
• Either:
  • Complete the form electronically then print.
  • Or print the form and complete it by hand.
• Sign your printed form by hand. (Electronic signatures not accepted.)
• Give your form to three members of The Mother Church who know you well to check over your application, and sign it as approvers.
• After all three have signed, mail your form to the Clerk of The Mother Church or scan and email it to clerk@christianscience.com.

That’s it! Once we’ve reviewed your application, you’ll be considered for the next admission of members, either in May/June or November.
